When the email is sent to the teacher....

An appointment has been applied for on Tuesday, 16 December 2008, 03:30 PM at
<%%TIME%%>,
by the student.....

The TIME parameter was missing in the object sent to the template. Also modified the date format to remove the time from that.

This bug has reoccurred since a later commit reverted the fix. See CONTRIB-1601.
