Description
Per a conversation with Martin D.
In the DB activity, if you have an item in the DB (like Moodle's plugin DB), and the owner modifies it, then the link disappears, and if there is a link to it, it reports 'item not found'. It should say something like 'this item cannot be viewed at this time' or something of the sort.

Petr, thanks for fixing this issue 
I tested the issue by adding an entry to a database activity with entries requiring approval, then accessed the URL when logged in as a guest. An 'Entry is not approved yet.' message was displayed.
I added more information about the error to http://docs.moodle.org/en/error/data/notapproved

This remains a problem on moodle.org.
Adding a 1.9.1 fix version in the hope it can be resolved soon.
