Give Moodle forms a consistent look - everywhere.

The formslib helps to offer users a consistent look and feel in all forms. Clean froms are much easier to understand and to deal with.

The question import form that you link to a screenshot of is linked up properly for me in Moodle 1.9.4+ using the standard theme (and it just uses standard formslib elements).

MDL-6937 is a specific list of forms that still need to be converted to formslib.php.

If there are specific forms that are inconsistent, please file separate tracker issues.
