Navigation link to gradebook from modules' report pages

Description

As explained by Gary Anderson in MDL-12122, although we can go from the gradebook to each module's grade report page, there are no reciprocal links on the module pages. The following modules should be improved with such links:

  • assignment
  • forum
  • glossary
  • hotpot
  • lesson
  • quiz
  • workshop

In addition, a grade.php page must be added to the lesson and hotpot modules, to redirect the link from the gradebook to their report.php page.

Um ... no! This was only supposed to happen on GRADING pages (ie assignment and quiz), not everywhere! see MDL-13075

With Martin, decided not to implement a link in the breadcrumb trail navigation, but to create instead a button on relevant pages. The location of that button is yet to be discussed

For now, just add it as a link like "See all course grades" on the right in the man part of the page, under whatever is already there. We can discuss it further later.
