[#MDL-14803] Rewrite question_delete_course and question_delete_activity to use the logic from question_delete_course_category that calls question_save_from_deletion - Moodle Tracker

This mitigates the impact of what I called Scenario A in MDL-14802.

This issue will help resolve:
MDL-24155	question_categories still has records after course deletion

This issue has a non-specific relationship to:
MDL-14802	It is possible to move shared questions in the question bank to a place where they are no longer properly shared
