moved form to top of the page. Seems better there, it is easier to control what is shown on the page with the form at the top, no need to scroll right down below the table.

moved form to top of the page. Seems better there, it is easier to control what is shown on the page with the form at the top, no need to scroll right down below the table.
Moved the form to the bottom of the page in HEAD branch for 2.0 but left the form where it was in the 1.9 branch. All forms are at the bottom of the page in 1.9 probably we will move them all to the top for 2.0.
