Haven't been able to duplicate the problem, but we originally thought this was due to a backup and restore done from an older version of Moodle to our installation of Moodle. We have since disabled our users ability to restore courses on our server, but the problem popped up again. The resources and activities that are created are incomplete. See the screenshots. Again, I haven't been able to recreate this, but it is still happening.
