Change the UI accordingly when shuffle questions is on

Description

When questions per page is set, the layout shown by the subtabs of the edit tab should not be that stored in $quiz->questions, but what is actually shown to the student.

Also, "Add question on this page" does not make sense when questions per page is set, and thus the question adding controls should be taken off the pages and moved to the beginning/end of the quiz.

Acutally, the issue is more about the shuffle questions option than the questions per page option.

I don't know why this is still open. I think the UI currently handles this OK. For example, when shuffle is on, the add buttons are only shown on the last page of the quiz.
