hello, exercise is not officially supported and is not part of official package
This looks like you did not have exercise in original 1.8x install and now for some reason the installation failed in 1.9.3, I would recommend deleting exercise module and the upgrade should complete fine.

Was the original 1.8.x installed from package or cvs? and 1.9.x?

The module is installed in both, 1.8.7+ and 1.9.3+.
I had install Moodle and upgrade from CVS.

Deleting module causes data lost (not cool, =|), but I can upgrade.

Phew – I had exactly the same issue – described here: http://moodle.org/mod/forum/discuss.php?d=117603. I thought it was just my situation.
I'm using a copy of a production database that runs with 1.9.3 code and loading in to a 1.9.4+ moodle test install. I could only fix the issue by removing the exercise code and dropping the tables. Check out http://www.earlham.edu/markp/miss/index.html#[[Exercise module -- solution]] for more details. Note that the 1.9.4+ install was working fine with a virgin database and exercise was the only built in module that gave me any gyp.
