[#MDL-18185] Regressions etc in gradebook improvements stage 1 HEAD commit - Moodle Tracker

going post here serious problems found in the mega commit, please fix asap
all issues here must be fixed before merging into STABLE

This issue will help resolve:
MDL-18083	META: Gradebook improvements - Stage 1

Progress

1.	do NOT check errors from $DB->update_record() ->insert_record() and friends	Closed	Nicolas Connault
2.	Gradebook tables must never be updated directly from a gradebook plugin	Closed	Nicolas Connault
3.	standardise meta hacks for IE specific CSS	Closed	Nicolas Connault
4.	do not pass grade item id around when editing cat - we already have category id	Closed	Nicolas Connault
