Several bugs found by a syntax checker

Description

Lars Kobbe, the participant of Developer Workshop in Bamberg Moot, reported a list of bugs found using an automated syntax checker (NetBeans or Eclipse? - I do not remember). I went thru the list and prepared the patch for 1.9. Now I am just going to prepare a merge and will commit. See the list and the resolution state attached.

Fixed at MOODLE_19_STABLE and merged into HEAD. I am going to check magpie bug upstream yet.

The bug in magpie reported upstream, a note added into README_MOODLE.txt.
The change in adodb is really very cosmetic and can be ignored during the next adodb upgrade.
