Petr, thanks for your fixes, which I tested by enabling xmlstrictheaders then viewing a grader report. I obtained the following error message (using Moodle 1.9.4+ build 20090505):

XML Parsing Error: mismatched tag. Expected: </td>.
Location: http://localhost/19/moodle/grade/report/grader/index.php?id=3

Fixed some more strict issues - works fine for me now, please retest and reopen if needed.
I did not find any </td> problem though, please add gradebook settings or course backup too, thanks a lot.

Petr, thanks for looking at this issue again.

Unfortunately I'm still getting the </td> error. It's also shown on http://test.moodle.org/1.9/grade/report/grader/index.php?id=16 (and for the grader report for other courses on the test site).
