New grade category don't load grade item defaults and don't validate min and max grades

Description

In the grade category creation, there was no grade item to get the grade_items default.
There is a patch attached that correct this problem. It also validates if grademax is greater than grademin.

Daniel, the second part of your patch (grade/edit/tree/category_form.php) duplicates the other patch (MDL-19429), so I am not including it.
