Description

If only one quiz attempt is allowed, then when the student reviews their quiz, they shouldn't see Attempt and under it #1. Since that is the only attempt, it should be indicated in some other way.

I would also like to allow the teacher to get rid of the Time taken column in that table because often quizzes are not time critical and this column might encourage students to think they have to do the quiz hastily.

Done. The checkin for this bug also fixes ~~MDL-5284~~, but I forgot to mention the other bug number in the commit comment.
