I see this is a bug listed as resolved, however in our new 1.6.2+, this appears whenever a random question is added to a quiz and you go to preview it with the little magnifying glass.
The quiz behaves normally.

MDL-6650 - Previewing individual random questions from the quiz editing screen was not working, and anyway, I'm not sure it makes any sense, so I removed that preview link.
