1. Login as a teacher, turn editing on in a course and add a survey activity.
2. Select ATTLS as survey type.
3. Click the 'Save and display' button and check that the Attitudes Towards Thinking and Learning Survey is displayed.
4. Update the activity and select 'Critical Incidents' as survey type.
5. Click the 'Save and display' button and check that the Critical Incidents survey is displayed.
6. Update the activity and select 'COLLES' as survey type.
7. Click the 'Save and display' button and check that the Constructivist On-line Learning Environment Survey is displayed.
