Several problems with help files

I made a completecheck of help files and found this issues:

These files contains only headlines, but no text

• workshop
  o usepassword
  o submissionstart
  o submissionend
  o assessmentstart
  o assesmentend
• quiz
  o formatmissingword
  o formatblackboard

Help files for these modules are in the package that are not standard packages or old modules

• questionnaire
• journal
  *exercise
  *dialogue
• attendance

Attendance and questionnaire should be deleted. They wasn't standards at any time. Journal, exercise and dialogue were standards in previous versions and can be part of the package forthe next time

There are several help files without healines i.e. in forum folder.
