Use the following steps to replicate the error:
1 - Create a new questionnaire
2 - Configure the first page and press continue
3 - Configure the second page and press continue
4 - Press Finish (yes, without creating any questions)
5 - Edit the questionnaire.
First Bug: Dates are not as saved. Year is 2009 instead of 2007.
6 - Try to add the first question (yes/no).
Second Bug: You get an error: Survey does not exist and you wont be able to complete this questionaire, never.
Third Bug: Each time you try to edit this questionaire, a new questionnaire wikk apear on Define New Content: - OR - Copy existing: area. And you will never be able to delete these questionnaires.

I no it is a stupid procedure, but this is the kind of actions a first user can do and can get discoraged to used the module.
