Template image incorrectly cached in browser, when different file uploaded

      1. edit an uploadpdf assignment
      2. click 'edit templates'
      3. upload a coversheet to use whilst creating the template
      4. close the window
      5. click 'edit templates' again
      6. upload a different file

      Result: original file (often) shown, instead of updated file, due to browser caching (refresh usually clears the problem).

      Expected result: new file shown
