      Description

      To make an anonymous questionnaire more "anonymous":
      a) once a questionnaire has been created as "anonymous", and responses have been entered, prevent questionnaire owner from reverting to "full name" type.
      b) if a questionnaire has been created as "anonymous", do not export submission dates to CSV, as those dates might reveal identity of respondent when compared with course log activity
