When questions are exported to Word table format, an extra table row containing instruction text is not included. However, when questions are created using the Word template, the instruction row is included. The Word template includes a simple facility to shuffle answers, but it isn't very smart, so it always expects an instruction row to be present. If this row is not present, then the facility only shuffles some of the answer options, not all.
