adding a "no answer" button to radio button questions

      Further to a request by Daniele in CONTRIB-774. I agree it would be a good feature to have a default-checked "No answer" button in all radio-button and yes/no questions. We already have a N/A (optional) button in Rate questions.

      Can Daniele and Mike please test the attached patch (for moodle 1.9) and comment on it.

      Please note that the appropriate $string['noanswer'] = 'No answer'; will have to be added to lang/.../questionnaire.php.
      I took the wording from a similar radio button in LimeSurvey. But moodle's Feedback module has a "Not selected" string. Which wording should we use?

      The extra "no answer" button only appears if the question is marked as "required". I think it does not make sense to display a No answer button if the question is "required".

      PS.- My patch includes a (minor) tidying up of the yes-no question type as well.
