Some improvements to grading in admin menu...

      Description

      Currently the admin menu shows this structure:

      Grade:
      ---> Grade settings
      ---> Reports:
      --------> Grader report
      --------> Outcomes

      I've found this (discussible) things:

      1) Don't understand where the "outcomes" element is under "reports". Shouldn't it go under "grade".
      2) If "grader report" is the only one element under "reports" perhaps it could go directly under "grade" too.
      3) The "scales" element is missing. I find it natural to have one link to scales there.
      4) As scales and outcomes are going to be inter-related. I would provide one link to jump between them or something similar.
      5) Each page should have one consistent "title" (big).
