Outcomes report is available even when outcomes are disabled

      When outcomes are disabled (i.e. the setting enableoutcomes is unchecked in Admin > Grades > General settings), teachers still select "Outcomes report" in their course gradebook and admins can still see an "Add an outcome" button in Admin > Grades > Outcomes (though perhaps it so they can create outcomes before enabling the feature).
