no distinction between graded & ungraded questions in manual grading report

      Description

      When manually grading quiz questions, a teacher is presented with a report that sets class="manual-ungraded" or class="manual-graded" depending on whether or not the question has a grade. Unfortunately, manual-ungraded and manual-graded aren't included in the standardwhite Moodle theme, so the teacher has no way to figure out which attempts are still ungraded. While fixing the problem by including these in the stylesheets would be fine, there should also be some indicator that does NOT depend on the theme currently in use. I've attached a patch that adds three asterisks to the link the teacher sees if the question has not yet been graded. This is simple, it is accessible, and it doesn't depend on the theme.
