Letter Grades are calculated using decimal numbers when 0 dec pl. have been specified. - resulting in conflicting values where rounding occurs.
Improve gradebook help files
Gradebook aggregation help files require rewording and/or renaming
There is an index page for grade helpfiles missing

Just noting also that grade/aggregation.html contains some typos.
aggregation2 has been removed (see MDL-13820)
Just noting that grade/enableajax.html is not used in 1.9.
Re. a grade help files index page, this could cause confusion as it would include help files from the 1.8 gradebook as well as from the 1.9 gradebook.
grade/grade.html, grade/gradecategory.html and grade/gradeexceptions.html are blank apart from a title.
grade/itemname.html and grade/keephigh.html have the same content.
grade/showquickgrading.html and grade/quickgrading.html have the same content.
decimalpoints.html suggested improvement:
Overall decimal points - Specifies the number of decimal points to display for each grade. This setting has no effect on grade calculations, which are made with an accuracy of 5 decimal places.
decimalpoints.html suggested improvement can help avoid grade letter confusion reported as MDL-13827
gradeletterdefault.html doesn't seem to be used
grep -iR "setHelpButton" *| grep "gradeletterdefault" gives no result
I removed the following files:
showquickgrading.html also removed
All issues resolved.
Reopening, as grade help files do not appear in the index of all help files.
Issues fixed in head, not yet backported to 1.9.1. Please check with head first.
