      Description

      Formal White theme display formats doesn't fit the Quiz Module in Moodle 1.9.
      I think it should be worked on for every format. See also MDL-13181 for "Continuous without author" (and Simple dictionary style?). Some of the css can be copied from the Standard Theme, but I am not sure if it can fix it all.

      My modifications looks like this (so far)
