$OUTPUT->confirm wraps the $message in <p> tags. That breaks if the message has two paragraphs.

      Description

For example, if you are logged into the course as a guest, and go to the view.php page of a quiz, the HTML does not validated because it $OUTPUT->confirm on like 109 with a two-paragraph message.

I think the confirm method in lib/outputrenderers.php should be changed to use a div. Also, all the other similar output methods should be reviewed to see if they suffer from the same issue.
