      The unit tests already test this pretty thoroughly, so I think we just need some basic testing through the UI.

      For on or two formats
      1. try importing an example file from question/format/.../tests/fixtures/..., using Question bank -> Import.

      2. Test importing some questions into the lesson module - which does not use the question bank.

      Description

      This follows on from MDL-20538. The constants that were removed from questionlib.php need to all be eliminated. (Only about about 150 references to them, according to grep ).
