Grade items not restored in 2.1 from a 1.9 backup file

      1. Prepare a course in 1.9 with some graded activities included (like assignment)
      2. Create several categories in the gradebook
      3. Move the activities grade items into categories
      4. Add some manually created grade items in the gradebook
      5. Define letter grades in the gradebook
      6. Define some outcome and link it with some module
      7. Backup the course and restore it in 2.1
      8. TEST: make sure that all the gradebook structures (categories, items, letters, outcome) are correctly transmitted to the restored course

      Originally we planned that the whole gradebook would be handled in the phase II of the 1.9 backups conversion. Unfortunately during the QA testing, it was detected that missing grade items lead to PHP notices - at least in the Assignment module.

      Let us try to convert non-user related torso of the gradebook to make these modules happy.
