Users's Notes in participants menu has faulty breadcrumb navigation setup and missing links

    Description

      Clicking the "Notes" link under the Participants link in the Navigation menu,
      Display the Notes page with problematic (faulty) breadcrams navigation links.
      Please see attached screen capture (or link)

      The Extra '''Participants / ► Notes ''' is redundant.
      and the "My Courses" is missing a link.
