Add missing version.php and pluginname string to some subplugins

      Note: Admin/dev with own test site required.

      1) Install 2.2.
      2) Go to the Admin->Plugins->Plugins overview page. All the plugins have version and proper name. No notices / warnings. **
      3) Install 2.1
      4) Upgrade to 2.2. In the page of plugins to upgrade, you get all the ones needing upgrade, all them with version, proper name, required Moodle version and some of them, other dependencies. No notices / warnings. **
      5) Repeat step 2) in the upgraded site **

        • There is one exception, and it's the randomsamatch qtype, it will be shown within square brackets. Ignore that in the tests, it's being discussed.

      Looking @ the Admin->Plugins->Plugins overview page, there are various plugins/subplugins missing version.php and/or the default "pluginname" lang string.

      This issue is about to add them to:

      • Text filters
      • Course formats
      • Profile field types
      • Site reports
      • Assignment / Assignment types
      • Database / Database field types
      • Database / Presets

      That will cause practically all having that information available but some (versions) are not going to be modified now:

      • Some authentication methods
      • Question behaviors
      • Question import/export formats
      • Workshop / Submissions allocation methods

      Perhaps, under discussion, we'll bump all versions and requires after all (nearer release).
