      Enable completion site-wide.
      Create a course with completion enabled.
      Enrol users.
      Check course_completions records exist in the database for those users.

      Create another course with completion disabled.
      Enrol users.
      Check course_completions records aren't created for these users in this course.
      Enable completion.
      Alter completion criteria.
      Check course_completions records were created for users in this course.

      Rather than scanning the entire user base every cron run, create course_completions records for users as soon as they enrol. And bulk recreate when completion criteria are modified, or set for the first time.
