Incorrect question state "not yet answered" in summary of progressive attempts when attempts build on previous ones

      Description

      If the quiz is set to allow more than one attempt, and the user does not change their answer to a question on attempt 2, the 'submit' screen says the answer is 'not saved'.

      Students think that they have to have all the 'answers saved' before they submit so its causing confusion, and some students are deliberately writing in wrong answers on attempt 2 to get the screen to say all answers have been saved. then they do an attempt 3 and change all the answers again... which is madness!

      Solution could be to change the language strings, or write something on the submit page to explain what is going on, or perhaps there is a tech solution that doesn't involve either suggestion.
