Moodle incorrectly counts questions in SCORM 2004 track data (see comment below) and unlike SCORM 1.2 it seems interactions do not match 1 to 1 with "questions", so the report algorithm would need to be overhauled to report on SCORM 2004 data.

SCORM 2004 (at least packages we've tested) can report multiple "interactions" for a single question, returning keys like cmi.interactions.N.learner_response. One must see the interaction's ID to verify which question it is, and it's not clear how/if Moodle should display that in the report (e.g. show only the interaction with the latest timestamp of all those with the same ID?)
