Move add buttons to the top of lists so they don't get pushed off the screen as lists grow

    Description

      There's a few places in Moodle, I'm mostly looking at the course and category editing pages here, where buttons to add new e.g. courses or categories are placed at the bottom of the list. Which works fine for short lists, but hides them as the the list grows longer.

      Current examples:

      http://192.168.56.10/moodle25/course/manage.php "add a sub-category", "add a new course"

      http://192.168.56.10/moodle25/course/category.php?id=1 "add new course"

      whereas some related pages do it the "right" way:

      http://192.168.56.10/moodle25/course/manage.php?id=1

      so there's an argument for consistency too.
