1. Create a quiz with Each attempt built on last set, and allowing multiple attempts.

2. Add an essay question, where the student may upload files, to the quiz.

3. Attempt the quiz twice as a student, ensuring that that responses from the frist attempt are used to populate the second attempt. However, note that any files uploaded on the first attempt will be lost. That is a separate issue: ~~MDL-39980~~. This change is about avoiding a fatal error that prevents the second attempt from being started at all.
