Please use Clean theme to test this.

Browse through feedback activity and make sure the pages contains:

<h1> displaying the course name

<h2> displaying the activity name (the name of the feedback)

<h3/xx> displaying the sub heading for the page.

Additionally, choose other non-bootstrap themes and re-test the above steps and make sure the page looks ok.

The description in overview page should be H3 not H4.

'Noting display' text in 'show responses' page should be H3.
