Fix heading level for Scorm activity

      Please use clean and non-bootstrap themes to test this issue.

      1. Add scorm activity to a course
      2. Go to file index page (/mod/scorm/index.php) and make sure the page contains activity name in h2.
      3. Visit the setting for activity within the course and admin, make sure the 'Display page name' option is not available.
      4. view the page and make sure it has:
      • course name as h1
      • activity name as h2

      Browse around the scorm activity and make sure there's no error occur.
