Add alternate name fields to "Lock user fields" in email enrolement.

      1. Setup moodle to use authentification with external LDAP source. (check https://docs.moodle.org/28/en/LDAP_authentication#Configuring_Moodle_authentication) for steps to configure LDAP with Moodle.
      2. Setup auth module to take some of fields middlename, alternatename, firstnamephonetic, lastnamephonetic from external source.
      2.1 You will need to enable LDAP in the authentication plugin area [Site administration ► Plugins ► Authentication ► Manage authentication].
      2.2 Click on settings next to 'LDAP server' on that page.
      2.3 Under 'Data mapping' make sure the additional name fields matches the 'middlename, alternatename etc.' fields that will need to be present on the LDAP server.
      3. Login with user, having such fields filled.
      4. Check for alternate name fields updated.
      5. Setup auth module to block editing of alternate name fields.
      6. Check user cannot edit them.
      7. Repeat 1-6 for external DB auth.
      8. Repeat 5-6 for email and manual auth.

      The administration page has a section for Locking user fields.
      The alternate name fields should be added to these fields.
