Question bank needs some CSS attention

      1. Using Clean theme and logged in as Admin go to a course and select Administration > Course Administration > Question Bank > Question.
      2. If the question bank is empty add a question.
      3. TEST to see that the 'action' icons, in the same line as the question, are visible and spaced evenly, but not cropped.
      4. If there is not one, add a quiz to the course, then go to Edit quiz, and make sure that the Question bank contents are shown.
      5. TEST that the action icons there also look OK.
      6. Repeat both TESTs using a non-bootstrap-based theme.

    Description

      The action icons are cropped on the right in Clean theme. But maybe we can style the whole table a little better (add padding or something), see screenshots.
