Clean Theme: Gradebook Edit categories and items interface is misaligned

    Description

      Steps to reproduce:

      1. Select clean theme
      2. Go to course
      3. Go to the grade book
      4. In the drop down select to Simple view (under Edit categories and items)
      5. Notice the huge amount of wide space on left side of table, seemingly a missing column, under 'name'.
