Strange alignment in 'categoryquestions' table.

      1. Create a course.
      2. Click on 'Question bank' in the 'Course administration' menu.
      3. Create a question.
      4. Once created you will be directed to the 'Question bank' page but now there will be a table displaying the newly created question.
      5. Notice the weird alignment (see attached screenshot).
