      • IE9, IE11, one other browser

      Look for:

      • error log problems
      • missing styles for core, themes or plugins

      There is caching logic in:

      • outputlib
      • csslib
      • styles.php
      • styles_debug.php

      We should simplify that so that outputlib only returns CSS and the caching appears in styles.php that serves the file.
