Create course, add glossary and quiz

As a student add several entries to glossary and make several attempts of quiz

As a teacher make sure you can see the new glossary entry and quiz attempt in recent activity report (click link from recent activity block)

Make sure that if you click on glossary entry name and quiz attempt name you go to this particular entry/attempt.

Regression from ~~MDL-44055~~
The SQL query now has two fields with the same alias

Quiz attempt id and glossary id are overwritten with user id which causes not-working links in the recent activity report.
