This applies to 2.4 Moodle. I have been told by Tim Hunt at the OU that this is bug.

Below is my default letter grade set at site level.
Site administration>Grades>Letter>Edit grade letters
Highest Lowest Letter
59.99 % 57.00 % C+
56.99 % 54.00 % C
Reproduce the problem
Set up assignment in usual way. Under >Grade use the default setting of 100 and > Simple Direct Grading.
Next step is grading.
Under > View/grade all submissions>Grade column>Grade>Grade out of 100>enter 57>save changes.
Current grade in grade book> C
Manual fix:
Under Settings>Grades>turn editing on>edit grade>final grade>enter 57.001>save changes.
It now shows in the grade report as C+
And from student view>settings>grades>user report it displays as a C+.
