Pix_icon should not have the same title as alt attribute

      1. Go to Site Administration > Courses > Manage Courses and Categories
      2. Using your favourite firebig clone inspect element on the show/hide visibility icon.
      This icon currently has it's title and alt attribute as the same text. This should not be the case (and this patch removes the title attribute as it adds nothing).

      Description

      Currently if no title is set on an image, it gets what is applied as the alt text for the image. For accessibility reasons this should not be the case.
