Need to tie up loose ends in Gradebook after MDL-43824

      Same as MDL-43824:
      This must be tested in both Clean and Base.

      You will require a course with several students and several gradeable activities (assignments etc). Make sure at least one of the gradeable activites is using a scale and at least one is not.

      1. Enable 'Static student' in Site Administration > Grades > Report > Grader settings
      2. View the grader report, check that the rows all line up when Static student
      3. Change the setting of grade_report_fixedstudents and repeat the check.
      4. Turn editing on (or off if you had it on) and repeat the test.
      5. Test on a smaller screen and on a larger screen (please note if any issues noted existed before this patch)

    Description

      There are a few loose ends that need some attention to detail.

      1. Line up the rows, with or without JavaScript enabled.
      2. Line up the rows, with or without the fixed column (Static Student) setting.
      3. Ensure there is sufficient height in the top right-hand scrollbar so that Mac users can see the scrollbar area if 'scrollbar' is in hidden mode.
      4. Remove redundant CSS from theme/base/style/grade.css
      5. Remove redundant CSS from grade/report/grade.css left over from MDL-43824
      6. Generally make the Gradebook look good and work perfectly!
