Quiz editing UI: in an empty quiz, the add menu is too narrow

      Please test a bootstrap and a non-bootstrap theme.

      Also, since this is CSS, you should probably test this on a range of browsers. I tested Firefox, Chrome and IE 8.

      • Find or make a quiz contiaining several questions.
      • Go to the Edit quiz page for that quiz.
      • Expand these menus, and make sure they look right (not too narrow).
        • The add menu for page 1.
        • The add menu after the last page.

      Several people reported it in the past: sometimes the add menu in the new quiz editing UI is too narrow.

      I think I just worked it out. It is just when the quiz is empty that this happens. Once you have some questions added, the problem goes away, which is why I thought I could not reproduce this.
