fetch_subscribed_users() docs do not indicate that additional checking may be required

As discussed in MDL-48090, the phpdocs for fetch_subscribed_users() needs to be updated to indicate that when adding potential discussion subscriptions in, the individual discussion subscriptions must be checked too.
